Yes, either by phone call or SM, the minimum limit is 500 on each card
Yes. Send them a private message asking them to combine and which one to keep open. They'll expect you to keep $500 on the card that will be closed and hopefully the account is over 1 year old. I've done this twice in the past.

Just a note that if the card you want to close hasn't been used in a while (either two or three statements at zero balance, I'm not sure which) they will sometimes let you move the entire credit line.
Last year I was able to move the entire $9k from my IHG to my Amazon when I closed the IHG.
I also recommend using private/secure messaging... it was painless.
