Speaking strictly from my own experience, I've never had a store card that upgraded to a V/MC without my being informed/asked about it. IOW, I've always received notice that my store card was eligible for an upgrade, or was being upgraded, to a MC/V. In some cases I could accept, and then lose the store card, in others the transfer happened automatically--but never without prior notice. I mean I never had an 'upgrade' card show up in my mail without previously being notified of the upgrade.
They must really want people to upgrade lol. I got the the Care Credit MasterCard in the mail a few weeks ago as well with no notice.
At this point in time, I am going to put the MC in the safe and decide if I want to accept the offer from Synchrony in March. I truly have no need to carry another CC and I am currently 4/5 for Chase. I would rather expand my Chase portfolio than accept an MC from Synchrony and muddy the waters. I will update in March as to what I decide if anyone is interested.

Thanks @Medic981 for providing the wonderful evidence. Hopefully you can update the community on whether you decide to take them up on the offer or keep your present card as is. Either way, glad to see you were targeted for it. I closed mine several months ago.
It's interesting how SYNCB has decided to approach these upgrades as of recent (at least for CareCredit MC) and I wonder if some of the other partnerships will have similar upgrade characteristics. Needless to say, we have concrete proof that it was still an upgrade, albeit an interesting method to solicit whether to accept or deny it.

I got one of these cards from care credit back in July but I don't know what to do. My CL on the regular care credit card is $20K. Offer expires 12/31/21. What to do?

That's entirely up to you @Anonymous.
Do you currenty use the CareCredit retail card often? I would say, given recent news that SYNCB no longer reports 'upgrades' as entirely new accounts, then there's definitely more versatility for having the MC and being able to use it outside of network providers who accept CareCredit. There's also targeted or promotional offers for some decent rewards when using the MC.
More than likely your limit will stay the same (unless the upgrade nudges it up a bit).
I didn't want to start a new thread, but I received the same upgrade offer to MC from Synchrony some time in April. I set the letter aside without reading. I originally thought my card was expiring since I did not keep up with the date.
This was unsolicited with no prior notice, offer, or communication through my synchrony account. It just showed up in the mail.
