So my credit card just showed up in the mail, and as if to add insult to injury, instead of [FIRST NAME] [LAST NAME] on the credit card, it's been printed [MIDDLE NAME] [LAST NAME]. Based on reading everyone's responses up to this point, I decided I wouldn't pursue any action and just live with it, but now I'm wondering if this error changes anything...

Again, IANAL, but I doubt this changes anything unless this somehow bolsters your case that it wasn't you signing those papers. But I don't think that's what you are arguing anyway. Is the tradeline reported on your CR? Sync is generally super quick to report new accounts. If it is showing up on the correct report then this may just be a mistake in printing. In any case, you may want to actually call them up and ask for a correction if they've got your info wrong.
OP do you still have your copy of the agreement?
Often times there are legal loopholes you can squeeze out of.
First I do want the intent to be that I empathize with this situation. Having stated that, I am curious as what you thought you were signing up for when a social security number and household income are both required fields before the instant application can be submitted. The cashier should have handed you back the application because at the bottom of that application is where one completes the information. If the cashier never gave you back the application that could be argument for deleting the account. You stated a slip printed that should have stated this is to signify that you understand you are applying for credit. If you did not sign that slip then the account is void and should be deleted.
The purpose of that slip is like the big flag waving and saying: Hey this is an application for consumer credit!! That signature is required before the cashier can press the submit button on the screen. If all this transpired without you gaining control of the situation, as unfortunate as it may be, you are the owner of a TJX Rewards Card. Hopefully you were approved for the Master Card. At least it can be used other places.
You know it just struck me that unless the credit line was opened fraudulently or without consent, there's really no "buyer's remorse" once you're approved is there? By that I mean even if you were to close the account the very next day, you'll still take a permanent hit to your AAoA until it ages off of your report 10 years later. So in some sense, getting denied could potentially end up being a good thing I suppose.
Well to finally wrap up this Odyssey, I decided to call and cancel the card. Maybe not the best idea, but I was so angry I just couldn't help it. Hopefully it doesn't hurt me too much in the long run. Live and learn I suppose. Thanks for everyone's feedback/responses. At the end of the day it really was completely on me and I have learned my lesson.
